This position will support the Human Services Coalition’s role in the Southern Tier Social Care Network. The Social Care Network is a group of community-based organizations, healthcare providers, and other partners who are working together to provide screening, navigation, and delivery of health-related social needs services to Medicaid Members (Medicaid beneficiaries). The Human Services Coalition is one of the organizations that is participating in the network.
